New Patient Registration

If you would like to register as a patient at this practice, you must reside within the practice area (Click here for the Practice Area Map) and be eligible for treatment under the NHS.

As a guide the practice area borders are:

  • North: Rochester Way
  • West: Penhill Road
  • South: Railway Line
  • East: Baldwyn's Park

You will need to complete various forms including an NHS Family Doctors Services Registration Form (GMS1), please bring it with you or you can request a registration form from a member of our reception team. You will also be asked to complete a medical questionnaire (which is useful as your medical records can take a while to arrive).

Please try and avoid our busiest times when you bring in your registration forms so we can devote time to you - please allow an hour for us to check your forms, take your blood pressure etc. Try and come in the afternoon and not on a Monday.

When you return the completed forms you should also bring proof of your address (e.g. a recent utility bill or bank statement) and if you were not born in the UK, your passport. If you are on any repeat medication please book an appointment to see a GP.

Medical treatment is available from the date of registration, not the date you bring in the forms. Please contact reception for further information.

Registration Forms

If you wish to complete your registration forms before attending the surgery, please click on the link below and print out the relevant forms.

When you bring in the completed forms to the surgery you should also bring proof of your address (e.g. a recent utility bill or bank statement) and if you were not born in the UK, your passport. We would also ask that you bring in a urine sample (specimen pots are available on request at reception) and if you are on any repeat medication please book an appointment to see a GP.
